I once had a metalic clunk which would occur one time only when going from drive after reverse, or vice versa. I checked the U-joints over and over and even had it in the shop a few times. Finally Randy (from our list) figured out it was the flange that attaches the propellor shaft to the transmission tail. The bolts had loosened just enough to allow relative motion between the two mating flanges when changing directions. Eventually, the bolts started to wear thin and the sound got more pronounced.
